I'm not sure what you mean when you say you used the "template option". When you use the template option in DW you create one page and it is used as a base frame for all your pages. After you do that, you create a page from that template and use the "save as" option...(index.html, etc.) I think every time you make a page DW goes back to the last option you used (template) rather than creating your page from the template.
As far as your css question...the link just needs to be correct.

In my opinion, given what you've described, I would start over (I know that sucks) and create your CSS style sheet rather than a template page. What you are presently doing sound like your mixing both styles and, frankly, it eliminates the advantages of using CSS since every page has to load your template setup, If you just use CSS you can include your menu as well as everything else in your template page without the download times.
You can create the CSS style sheet then create a basic page which links to that with no content on it, just your site layout. Then when you want to create a new page, open up that page and save it as page1.html, page2.html, etc. When you save it as something else it will not delete that page, it remains the same.
I've probably just confused the issue to no end, sorry. If you would like, send me some links to what your working on and I can put together the CSS sheet and the html template page for you this weekend. You can then just save them in DW and use them as I've described. (I'll send you clearer instructions with them.) It will only take me about a half-an-hour to set it up for you and it'll save you hours of headaches.
