I usually start by drawing it out on paper, boxes of where I want certain content to appear. After that I create style.css, header.php, index.php and footer.php files. Then I start making the main div containers, usually wrapper for the main container that sets a width to the layout, header for the header content, container for the main content and footer for copyright information and privacy/contact links. Then I move on to Photoshop and Illustrator to create the graphics, save them and start coding the stylesheet.

Don't hate me but I always prefer to look online for a free template with the cloest structure to what im looking for. Then the main code is already done and there is a high chance it looks good in every browser. From there you can tweak it as much as you like.

Actually that is a really good way to learn, especially when using a visual type editor. It gives you a chance to see actual elements in action and take it apart to make it look the way you want. You will learn faster...

Yeah that's how I learnt with Dreamweaver. Then I moved on to notepad++ because it's so fast to use.

I started off my last site, with Lisa's basic 3 column layout. I then researched about a half a dozen sites that I really liked and tried to incorporate some of their ideas into my own. Even while making my layout, if I came across and idea or another layout, I would go back and tweak it again.
I have in the past, put my basic layout on paper first, but it always seems to be the same. Header, columns, content, footer, etc. If I decide the site needs to be more complex, I would probably layout it out on paper first.
