Advertising space rates
I haven't adjusted my rates in years for my paid advertisers. Is there a guide anywhere as to what the going rates should be for certain amounts of traffic?

Do you use AdSense or any program that tracks your eCPM (earnings per 1,000 impressions)? If so, you can take your eCPM number and use that. For example, if it's $2.00 then that means you earn $2.00 for ever 1,000 impressions. So if your site gets 5,000 visits per month you would charge $10/month for the ad. That's one way of doing it.

Gotcha. Yes, I have adsense on there.
I have rotating banners at the top, how do I account for the randomness of the display? I also have smaller, static banners on the side too.

I don't think paying advertisors would be too happy having their ad on a rotating banner.
It may be best to use affiliate links on the rotating banner and have your ad spaces somewhere else on the page or replace the rotating banner.

Well, that is actually part of the package that they sign up for, along with the static, all the time side banners. They also get weekly news posts too.

You could do a quick Google search for "ad server" then when you go to ad server sites, you would click on publisher. Most have online help or customer service numbers, they are really helpful, and will tell you what you can expect.
Just a thought.
