Default MKL question

I am one of those who struggled to understand BRAINSTORM IT in the beginning. Now that I have 30+ pages written that I feel are all geared to information that people are seeking in my niche and see my traffic growing slowly but surely....very slowly.....I have gone back and tried to better understand BI and MKL so that pages I write from here on are all geared toward keywords that people are researching in my niche.

These are my questions:

Is MKL updated periodically and if so, how often. What I mean is, if a keyword had high demand, low supply, a year ago when I did my first BI, doesn't that change over time?

And also, is there a reason to put keywords in your content even if they are not the main topic of the page. Here's an example:

running to lose weight 2446/215
lose weight by running 1073/93

would it be of benefit to name the page "running to lose weight" and also add a few phrases within the content "lose weight by running"? or do spiders only look for the primary keyword of the page.

I have researched this on the SBI forum, but usually I just end up getting more confused because they either "talk over my head" or different people give different answers.

Thanks in advance for any help anyone here can give me
