Usually, it depends on the niche. For example, ads related to webhosting pay the best. Because of smart pricing, the ppc has decreased and is decreasing gradually.
Yes people really do click on the ads. The ads are relevant and unless inspected closely will not appear like ads to the layman. Bigger ad blocks convert well for this reason as the fact that they are ads is not obvious.
